Dental Pain after tooth extraction. Going back to dentist is complicated?

I had surgery done on the sixth for a lower left tooth extraction. The dentist sent me home with ibuprofin and hydrocodone. Well, those two are gone and after the numbing wore off, Even the hydrocodone wasn't enough. The pain is almost unbearable and always constant with occasional throbbing. I moved counties and can not go back to the original dentist that performed the tooth extraction, plus i'm very poor and do not have the gas either to go 50 miles to the only dentist that takes state and government insurance if i still was in that county. The pain is so bad!!! The dentist told me to quit with the cold and start with the heat compresses. This pain clouds my thoughts and makes me so irritable at everything!!! plus, I'm so sick with so much that its all quite intolerable. i went to the E.R and my PCP. They said theres nothing they can do for dry socket. And I was wondering if it was normal to have such a huge and deep hole at the extraction site still after so many days passed. And if it was normal to use a drill to get the part of my tooth that was stuck in my gums after the majority snapped and broke off. The headaches won't go away, Dr. says I now have sinusitis. There was a deep infection in which I took antibiotics for way before the surgery. like a month or so before. I tried ambesol and packing it myself with clove oil. I'm to the point of constant tears and irribility from agony now. in a scale of one to ten, i give this an 8!!!!!!! and I am one to have an exceptional pain tolerance. Please, does anyone have any helpful suggestions or advice?
